We are one of Nevada’s largest mobile physician groups. We’re looking for a Full Time NP to join us in Las Vegas.
Visits will not take place in a traditional clinic setting. The NP will be a mobile PCP and will have the support of office medical assistants and additional services such as mobile imaging, labs, etc.
The Nurse Practitioner will conduct primary care patient visits in independent and assisted living facilities as well as residential homes in an assigned geographic area of responsibility. They will conduct examinations independently with excellent clinical competence. The Nurse Practitioner will work collaboratively with the patient, family, and patient’s residential facility to develop individualized plans of care.
Duties to include but not limited to:
- Conducts medical evaluations of predominantly elderly, frail, chronically ill homebound patients in their home environment.
- Documents visit via EMR for medical record and billing purposes.
- Develops a treatment plan in collaboration with the patient and family and their goals of care.
- Evaluates patient medication and writes/orders prescriptions as needed.
- Orders and interprets diagnostic tests relevant to patient’s needs.
- Collaborates with multidisciplinary team members.
- Facilitates the patient’s transition within and between health care settings.
- Educates and counsels patients and their families regarding health care options.
- Makes appropriate referrals to home health or hospice as needed.
- Facilitates access to appropriate supportive care services.
- Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Rotating On-Call schedule (Phone triage after hours) 1 week on call, rotating between providers. (on call once every 7-8 weeks)
- Enhance the health care practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
